If you are covered through Medicaid , Please call Medical Answering Services ( MAS) at 1-866-932-7740 and request for " Angels Heart Medical Transportation " for scheduling your appointment. 

You can also book a ride online by creating an enrollee account with MAS  at https://www.medanswering.com/enrolleeLogin.taf

If you are not covered through Medicaid and need services please call us during our business hours. We accept debit/credit, cash, and personal checks

• Date of Birth
• Address
• Contact Number
• Medical Reasons for Transportations
• Level of Transportation Required
• Name of Ordering Physician and name of physician that will be seen
• Date and time of appointment
• Location of the medical appointments ( this is key) including full street address
• Ask for " Angels Heart" for your ride
• Any Other Special instruction needed for the trip
